Referral
Pre-screens eligible homeless veterans, prioritizes greatest needs, and streamlines placement into permanent supportive housing.
Intake
VA-referred veterans receive personalized intake, recovery planning, and dedicated case management with a 1:6 case manager-to-veteran ratio.
Placement
Provides emergency stabilization and permanent supportive housing, creating a foundation for veterans' recovery, stability, and lasting independence.
Graduation
Residents receive ongoing support to promote recovery, independence, and successful community reintegration.
